Our Cancer Care Coordinators are a single point of contact, not a clinician. 

Our role is to assist patients with their cancer care. Assisting on delivery of the DES; Improving Cancer Screening rates, addressing health inequalities, providing reviews for pts with cancer, supporting MDT meetings and carrying practice level Cancer related admin

We work closely with the GPs other primary care professionals within the Primary Care Network (PCN) to identify and manage a caseload of identified patients, ensuring the appropriate support is made available to them.
