Referrals can be made by EMIS task or via email. Our Mental Health Care Coordinators are a single point of contact, not a clinician. Our role is to assist patients with their mental health and wellbeing. We provide extra time, capacity, and expertise to support patients who require additional support with their mental health we aim to provide a joined-up and coordinated care journey for patients. We work closely with the GPs other primary care professionals within the Primary Care Network (PCN) to identify and manage a caseload of identified patients, ensuring the appropriate support is made available to them.
Mental Health Care Coordinator reviews or support GPs to review (as appropriate to individual patient) to optimize mental health care of patient (biopsychosocial aspects) on SMI register who are not currently under Secondary care (Pennine Care) and work to identify hard to reach patients who have not attended for annual SMI mental/physical health review and adopt assertive outreach approach to establish needs/engagement.
