A LD Care Coordinator is a single point of contact, not a clinician. 

The aim of the Learning Disability Care Coordinator role is to support and safeguard our Learning Disability patients and their carers and help them to receive the best and most appropriate care and support services.

Our Care Coordinators are here to complete the patients Annual Learning Disability Health Care checks for patients aged over 14, and make referrals to other services as and when required.
